Hospice Care
Personalized
Patient Care
At Patriot Healthcare, our hospice services offer four levels of care designed to meet the unique needs of each patient and family during their end-of-life journey.
Each level of care is tailored to provide compassionate support and comfort when it’s needed most:

Routine Home Care
Routine Home Care allows hospice patients to receive compassionate care and symptom management in the comfort of their own homes. Our skilled team of nurses, aides, and medical social workers visit regularly to provide medical care, pain relief, and emotional support; while ensuring patients and their families have the resources they need. This care focuses on improving quality of life, allowing patients to remain surrounded by loved ones in a familiar environment.

Continuous Care
Continuous Care provides round-the-clock support for hospice patients experiencing acute symptoms that require more intensive management. Our skilled nursing team stays by the patient’s side, delivering hands-on care in their home to stabilize symptoms and ensure comfort. This temporary, short-term care is designed to keep patients comfortable during periods of crisis, offering peace of mind for both patients and their families.

Respite Care
Respite Care offers temporary relief for primary caregivers by providing short-term inpatient care for hospice patients. This allows caregivers to rest and recharge while knowing their loved one is in a safe and comfortable environment with 24/7 professional support. Respite care ensures that patients continue to receive the high-quality, compassionate care they deserve while giving caregivers the time they need for self-care.

General Inpatient Care
General Inpatient Care provides short-term, intensive care for hospice patients who need symptom management that cannot be effectively managed at home. This level of care is delivered in a hospital, hospice facility, or skilled nursing facility, with a focus on stabilizing severe symptoms such as uncontrolled pain or breathing difficulties. Our goal is to bring comfort and relief to patients during times of crisis, ensuring they receive the highest standard of care until they can return to their home setting.
