Patriot Hospice & Palliative Care was founded to provide comprehensive, compassionate care for patients facing serious and terminal illnesses. Originating from Patriot Homecare, established in 2012, the organization expanded to offer hospice and palliative care, addressing both medical and emotional needs of patients and their families.

In 2022, Patriot launched Patriot Cares, a nonprofit providing resources not typically covered by insurance, such as medical equipment and medications. This initiative also fulfills the final wishes of hospice patients, emphasizing compassion and integrity in delivering exceptional care.

We offer comprehensive hospice and palliative care services, including home-based primary care, routine and acute care visits, palliative care with added benefits like social worker and chaplain access, and hospice care focused on comfort and symptom management for terminally ill patients.

Hospice care is specifically for patients with terminal illnesses who are no longer seeking curative treatment and focuses on comfort and quality of life. We offer comprehensive support for families, including bereavement services for up to 13 months after the patient’s passing, volunteer assistance, and respite care to give caregivers a short-term reprieve​.

Patriot Cares, our nonprofit organization, provides resources and services not typically covered by insurance, such as medical equipment, medications, food, and utilities. We also fulfill the final wishes of hospice patients, providing meaningful experiences during their last days​.
